FOR CARE PROVIDED IN TENNESSEE, WE PROVIDE NON-OPIOID BASED INTERVENTIONAL NON-OPERATIVE MANAGEMENT FOR BOTH NEW AND OLD (CHRONIC) CONDITIONS INCLUDING SPINE JOINT AND PELVIC PROCEDURES AND OTHER RECOMMENDATIONS AIMED AT IMPROVING PAIN ASSOCIATED WITH THE CONDITIONS. WE DO NOT CURRENTLY PROVIDE CHRONIC OPIOID THERAPY.
We are so happy and honoured to be seeing you soon. Please choose and complete the form below that corresponds to your upcoming visit with us. In some cases, there may be more than one form to complete. We will make every attempt to notify you in advance of your scheduled visit if a special or additional form needs to be completed. Our group appreciates forms being completed 5 days prior to your visit with us. This allows for advanced preparation and significantly decreases your wait time. Please retain the confirmation numbers associated with the successful submission of your on line form. We are NOT able to see clients who have not completed the intake process in the manner described-5 days prior to your visit. Also, patients who are being evaluated for our chronic pain program who fail the first toxicology (drug) screen, will NOT be eligible a trial of opioid (narcotic) management

IMPORTANT NOTE: We appreciate your consideration and attention to the wearing of appropriate attire to your appointment including: NO HOUSE SHOES/SLIPPERS NO PAJAMAS and PANTS MUST BE PULLED UP TO THE WAIST. NO UNDERWEAR SHOWING. Please wear clean non-revealing clothing. This helps to create a comfortable atmosphere for everyone.
Also, we would like all prospective chronic pain management patients to know that we do require a journal (pain diary) of some sort be kept regularly-Please review the journal tab here on the site for a more complete explanation. Additionally, we do perform screens (by mouth or by urine) for the purpose of acceptance into our programs; prospective patients are required to "pass" these screenings from the very first test forward. We certainly appreciate the importance and the value of each of our patient's time, so please be sure to consider these requirements when desiring to participate in our pain management programmes.
